Forbearance
Mandatory Forbearance Request – Student Loan Debt Burden
This Mandatory Forbearance Request Form may temporarily postpone your payments or allow you to make smaller payments than previously scheduled if the monthly payments due on all your Title IV loans are equal to or greater than 20% of your total monthly income. The forbearance may be granted for up to 12 months at a time. Maximum eligibility for this forbearance is 3 years. Before you utilize this request form, please contact our Customer Service Department toll-free at 1-800-366-4372. Your student loan holder needs to be aware of your situation in order to provide you with the appropriate information and instructions.

Mandatory Forbearance Request – Medical or Dental Internship/Residency Program, National Guard Duty or Department of Defense Loan Repayment Program
This Mandatory Forbearance Request Form may temporarily postpone your payments or allow you to make smaller payments than previously scheduled if you are engaged in a medical or dental Internship/Residency Program, a member of the National Guard, or performing a type of service that qualifies you for partial repayment of your loan(s) under the Department of Defense Loan Repayment Program. The forbearance may be granted for up to 12 months at a time. Before you utilize this request form, please contact our Customer Service Department toll-free at 1-800-366-4372.

Mandatory Forbearance Request – AmeriCorps Service
Under the National and Community Service Trust Act of 1993, borrowers serving in approved national service positions in AmeriCorps qualify for mandatory forbearance during service, which postpones the member’s obligation to make loan payments. AmeriCorps members will need to complete one form for each service period. After completing the Member section of the form, give the form to your program director or state office official who will certify and forward the request to your lending institution or loan servicer. You must complete one form for each loan holder.

Teacher Loan Forgiveness Forbearance Request
Use this form to request a forbearance of your loan payments in annual increments during each of your qualifying years of teaching service . To qualify for this forbearance, your loan holder will need to determine that you will qualify for forgiveness of your entire loan balance, plus interest, that will be outstanding at the end of your five years of qualifying teaching service. (Available to new borrowers after October 1, 1998.)

Economic Hardship Deferment Request
Deferment request for the qualified borrower who has been granted this deferment by the William D. Ford Direct Loan Program, who is receiving federal or state public assistance, who is serving as a Peace Corps volunteer, or who meets specific low income qualifications. (Available to a borrower who has an outstanding balance and all loan(s) were made on or after July 1, 1993.)

Military Deferment Request
The Military Service Deferment is for borrowers serving on active duty military service, or National Guard duty, during a war or other military operation, or a national emergency and for an additional 180 days following the completion of the qualifying military service.

Parent PLUS Deferment Request
For Parent PLUS loans first disbursed on or after July 1, 2008, the parent may request that these loans be deferred while the parent or their student is enrolled at least half-time and for 6 months after such enrollment ends.

Teacher Loan Forgiveness Application
The Teacher Loan Forgiveness Program is intended to encourage individuals to enter into and continue in the teaching profession. Use this form to apply for your applicable forgiveness amount after you have completed your five years of qualifying teaching service. (Available to new borrowers after October 1, 1998.)

Total and Permanent Disability Discharge and Veterans Disability Discharge
If a borrower is unable to engage in any substantial gainful activity by reason of a medically determinable physical or mental impairment that can be expected to result in death, has lasted for a continuous period of not less than 60 months, or can be expected to last for a continuous period of not less than 60 months, the borrower may apply for a total and permanent disability (TPD) discharge of his or her Federal Family Education Loans (FFEL), Perkins Loans, William D. Ford Federal Direct Loans (Direct Loans), or Teacher Education Assistance for College and Higher Education (TEACH) Grant service obligation.
